Monday through Friday, 8:00 AM – 5:00 PM.

Weekend and after-hours appointments may be available — call to ask.

Insurance & Payment

We accept most major insurance plans. Our front desk team will verify your coverage before your visit so there are no surprises. Self-pay rates are available for uninsured patients.

Location & Parking

We are located at 2 Medical Plaza Pl, Minden, LA 71055.

Complimentary parking is available in the medical plaza lot. The building is wheelchair accessible.

What to Bring

Photo ID, your insurance card, a list of current medications, and any referral forms your specialist may have provided. Arrive 10 minutes early to complete check-in paperwork.
